I've restrained myself from weighing in on this one. I could go on for a number of days, sharing war stories and experiences.
I would offer four resources, however, that may not have been mentioned yet.
At UC Davis, we use the Campus Design Guide here:
http://dcm.ucdavis.edu/cdg/index.html I've had a fair amount of input into the document and it gets well-used.
The UC system has a Laboratory Design Guide here:
http://lsdm.ucop.edu/ It's an update of our old lab safety design guide from 2007.
DiBerardinis and Baum have published the 4th edition of Guidelines for Laboratory Design: Health, Safety, and Environmental Considerations. It's

The Harvard School of Public Health offers and advanced laboratory safety design workshop here:
https://www.hsph.harvard.edu/ecpe/programs/guidelines-for-laboratory-design/ I've attended it and it's really worthwhile.
